The Mick Thompson KIT (untrimmed, unpainted) vacformed mask shell is $29.99 plus shipping.
The deluxe kit includes HAIR and Elastic for dressing the mask.  That kit is $38.99

SID DEMON MASK
The DJ (Sid) Mask is seen here:
 
It has been reproduced and is available either as a wall display or a wearable rubberized mask.
The wall display is $60  the rubberized mask (cast with an elasticized polymer from an rtv mold) is $60 untrimmed and unpainted or $79 trimmed and detailed.

Sid Mask: $39 for a vac-shell kit.  $45 for the vac shell, hair and elastic deluxe kit.
